New Patient Instructions

1.Patients must bring their mammogram and/or ultrasound and/or breast MRI images to their initial consultation. You must contact the hospital or facility and request the images printed on film. They sometimes will provide an electronic version on CD-ROM. (Our office cannot request these confidential records in advance of your initial consultation)

2.Bring your Insurance ID card.

3.Bring your “Co-Pay” payment.

4.Arrive 15 minutes prior to your appointment to complete registration documents.

Download, print, and complete both the “Registration” and “Medical History” forms before your visit. Completing forms at home can significantly shorten the waiting time required to see your surgeon.

Insurance Instructions

1.Always call the Member Services number on your insurance card to confirm our participation with your insurance plan. Call us for help if it is unclear that we participate with your insurance.

2.Bring your insurance card with you to your appointment.

3.All patients are responsible for obtaining required referral or authorization documents, if needed. Referral or authorization documents from primary care physicians are often needed for HMO insurance plans. It can often take several days to acquire a referral or authorization from your referring physician or insurance plan. We will not be able to evaluate patients if the appropriate referral or authorization documents are not in order. 

4.We can evaluate and treat patients without health insurance. Patients are financially responsible for charges of all services performed. Call or speak to our billing staff for further details.

5.All patients are financially responsible for co-payments or co-insurance at the time when services are rendered.

6.We can often evaluate and treat patients who have insurance plans with whom we do not participate. We must verify “Out-of-Network” benefits prior to providing any services.
